An agreement has been confirmed with Care United to do Athletic Physicals, May 18th, for a discounted rate of $15.
This will be a one day special arrangement to benefit the community of Forney.
They have graciously agreed to bring in the additional staffing needed to take care of the district’s need for all students entering 7th through 12th grade that will be involved in an extracurricular programs that requires a physical evaluation.
The parent portion of the physical packet will have to be completed prior to arrival at Care United and a parent will have to be present at the location.
To simplify the process such that there will not be a large mass showing at one time we are requesting an alphabet divisor.
Care United will open at 8:00am to start the physical evaluations and will continue throughout the day until 6:00pm.
